When you have been asked to perform extra work and a request is made that you furnish a quote. When you are considering whether to do the work, you first asked for more information as to the extent. You then decide if it is worth going forward at that price. You start by sending this sheet to them. After this sheet is filled-out and sent back, you then decide to approve or not. If completely approved, then fill-out and sign the “Acceptance” portion at the bottom. Return it to the company asking for the extra work. If rejected, sign and date under “Non-Approval—a)”. If some of the quote is acceptable but not all, fill-out and return under “Non-Approval—b)”.
