For large and more complicated jobs in which many change orders and requests for change orders have been sent to the general contractor/owner/architect/project manager for review. In such cases, they are either 1) approved as is, 2) approved as revised, 3) not approved, 4) a request has been made to revise and resubmit, or worse yet, 5) pending and sitting on someone’s desk. This form keeps track of the status of the change orders. It also allows you to track how long it is taking to get approval, monitor when you are getting paid for them in the following progress draws, and reminds you to request or demand approval if there is a contest in this regard.
